Responsible for collaborating with a variety of agencies and service providers in the community on both a professional and paraprofessional level to provide housing and supportive services for program participants. Responsible for providing recovery based, client centered support services using the most effective available resources in meeting the identified service plan goals for each client.

Required Education and Experience: Bachelor's Degree in a human service related field preferred and one to three years of experience in direct services with individuals with disabilities. The individual must possess a working knowledge of community resources and excellent communication and interpersonal skills. Computer skills and knowledge of Microsoft desirable.

While performing the duties of this job, the employee can sometimes be exposed to weather conditions. The noise level in the work environment is usually moderate.
