UnitedHealth Group - - Responsibilities: Manage patient referrals and visit schedules using an automated scheduling system; Assign patient assessments and other visits as ordered by the physician; Collaborate with the Team Leader to identify clinicians with appropriate experience and skill set; Monitor pending referrals daily and assign licensed professionals and case managers for start of care visits; Communicate daily with field staff regarding visits that are late, pending, or incomplete and resolve as appropriate
