Responsible for the management and operation of the Department of Pharmacy in accordance with the hospital's stated philosophies, goals, and objectives to ensure the consistent provision of high-quality patient care.
Responsibilities
Assures that the highest quality of employees is hired, and appropriate licensure is maintained.
Coordinates the daily activities of the pharmacy department including the scheduling of necessary staff to carry out pharmacy services.
Receives and researches all requests from health care professionals for drug information and provides written/verbal response.
Cultivates and maintains good working relations with the Medical Staff and other department personnel.
Coordinates and participates in the initial training/orientation of new pharmacists and technicians.
Performs mandatory review of each employee's performance in a timely manner.
Participates in the formulary review process by developing drug monographs, related data collection, and recommendation for submission to the Pharmacy & Therapeutics Committee.
Coordinates reporting of Adverse Drug Reaction reports on a quarterly basis to the Pharmacy & Therapeutics Committee.
Ensures compliance with State and other regulatory agencies such as the JCAHO, DEA, FDA, etc.
Works in conjunction with all Nurse Managers to ensure efficient and accurate distribution, storage, and security of all pharmaceuticals in areas outside of the pharmacy department.
Attends and participates in all assigned hospital committees.
Reviews departmental systems, effects changes as needed to improve services and simplify workflow.
Monitors workflow of the department; promotes harmonious working environment; encourages staff involvement in improvement for the department.
Follows policies and procedures in the seven areas of the Environment of Care to include safety, security, hazardous medical/waste, emergency preparedness, life safety, medical equipment and building utility systems.
Understands role in maintaining standard inventory PAR levels.
Understands reorder points (based on PAR levels).
Understands process for ordering product through wholesaler.
Understands process for ordering product direct from manufacturer.
Describes monitoring process.
States process for adding items to 'Want Book.'
Demonstrates ability to run shelves for re-ordering product.
States hospital/department policy and procedure for ordering.
Demonstrates ability to place order using the ECHO system.
Demonstrates ability to check for appropriate contract and generic substitutions in the ECHO program.
States necessary lead times for ordering items.
Hospital Wide Requirements.
Observes department/hospital policies concerning smoking, telephone use, dress code and other rules governing conduct while at work.
Meets hospital wide and department specific educational requirements. Attends all annual mandates / required in?services: (i.e. CPR, Safety, infection control), CPR current if required. Maintains a current Employee Health screening status as per hospital policy.
Utilizes hospital/departmental manuals appropriately. Knowledgeable of location and use of emergency response equipment. Responds to Emergency Preparedness Plans according to hospital policy.
Attends and participates in a minimum of 3/4ths of required meetings such as departmental or committee meetings.
Employee demonstrates an understanding of and adherence to the Code of Conduct.
Maintains a safe and orderly work area. Cleans equipment as needed. Maintains equipment in safe and proper working condition.
When indicated, completes occurrence reports in a timely manner.
Complies with and helps to enforce safety and health policies. Applies appropriate ergonomic principles to reduce the potential for job related injuries.
Implements all policies and procedures to identify and reduce the risks of acquiring and transmitting infections among patients, employees, and others.

Develops and implements policies and procedures that guide and support the provision of services.

Recommends a sufficient number of qualified and competent persons to provide care or service.
Exhibits effective time management / organization skills, including prioritizing and delegating.
Plans & develops an annual operating and capital equipment budget based on input from employees, physicians, & others.
Monitor financial reports, identifies variances, plans and implements opportunities for improvement. Assesses outcome of action plan. Completes monthly DOR variance report.
Determines the qualifications and competence of department personnel. Ensures and documents competency of staff including age specific competency.
Provides new employees with a complete orientation process that includes feedback from primary preceptors.
Provides timely Initial and annual performance evaluations.
Provides and communicates evaluation of staff performance on an ongoing basis and at scheduled performance evaluations. Complete including all attachments, examples, specifics and goal setting with staff.
Recruits, interviews, selects, and hires appropriate employees in cooperation with Human Resources. Turnover rate: _
Plans and provides in?service training and continuing education of all persons in the department. Utilizes internal and external resources, as appropriate.

Qualifications
Experience 5 years as pharmacist.
Education Graduate of Pharmacy Program.
Special Skills Must be highly proficient in the management skills of planning, controlling, coordinating and measuring departmental activities as they relate to hospital pharmacy.
Licensure Unrestricted license through Louisiana Board of Pharmacy.
