DSW SUPERVISOR JOB DESCRIPTION

Hours: Monday-Friday 8:00 4:30 pm. Every other Friday off at noon, unless needed for job commitments.



EDUCATION:


High school diploma required, some college preferred.


Any healthcare education a plus.



EXPERIENCE:


MUST HAVE WORKED AS A SUPERVISOR FOR A MINIMUM OF 1 YEAR. PREFERABLY IN THE HEALTHCARE INDUSTRY.


OTHER SUPERVISORY EXPERIENCE A PLUS.

Skills that can assist DSWs &/or clients with any issue, able to diffuse the situation by listening completely to their concerns before speaking.


Must have reliable transportation, valid drivers license and safe driving record


Agree to preserve the confidentiality of all clients, their families, staff management & company information


Experienced at using a laptop, emails, scanning, copier & fax


Must adapt quickly to new computer programs.



JOB FUNCTIONS/RESPONSIBILITIES:


Conduct in-person visits with each DSW, required to evaluate performance, client status changes, and emergency drills conducted to ensure services provided per ISP requirements, annual and as needed.


Supervisor must verify DSW visits daily to ensure services are provided in accordance with the ISP.


If DSW positions cannot be staffed, the DSW Supervisor must report to work.


Supervisor is responsible for answering after hour & staffing calls on rotation.


Setting up client binders, check yearly.


Supervisor must complete accident/incident reports within 24 hours. Report all accidents/incidents & complaints to Manager ASAP. Also, the Home Care Supervisor must complete accident/incident follow up reports 3 days thereafter.


Create schedules for DSWs, initially, annually and as needed.


All client & employee documents must be scanned into data base and filed immediately. Conduct daily PA checks to ensure all clients have a valid PA.


Handle filing, generating reports, answer phones, ordering supplies.


Immediately report any indication of a potentially significant issue developing to Manager.


Retain proper documentation regarding all client and staff communication


Ability to understand and fully carry out, instructions given by Director or Manager.


Must sign in and out on the Sign-In Sheet.


Add all events to calendar in database.


You will be a part of our team, therefore, you may be asked to attend & assist in fun community functions, such as: trade shows, fairs or other company events along with other office personnel.


Keep open lines of communication with office staff.


Any additional job related tasks as assigned by Executive Director
