Catholic Charities of Acadiana is seeking a compassionate and mission-driven Case Manager to join our team. The Case Manager provides tailored case management services to individuals experiencing crisis, ensuring they receive the care, resources, and advocacy needed to achieve stability and long-term success. Key Responsibilities

• Conduct intake assessments to identify client needs, goals, and challenges.
• Develop and implement individualized care strategies focused on stability and risk reduction.
• Provide intensive care coordination, including consistent follow-up for up to 6 months post-program.
• Advocate for clients to access services such as healthcare, mental health and substance use treatment, housing, employment, and financial assistance.
• Monitor client progress and maintain accurate, timely case documentation.
• Ensure appropriate referrals and follow-up with internal and external service providers.
• Participate in staff meetings, trainings, and case conferencing.
• Uphold the mission and values of Catholic Charities of Acadiana through every interaction.
• Adapt to unpredictable situations with professionalism, flexibility, and calmness.
• Conduct client outreach in the community and work flexible hours, including evenings or weekends, as needed.

Job Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology considered with related experience in human services field required; Master's degree in Social Work, Psychology, Sociology preferred
• Experience working in a shelter or housing program environment preferred
• Strong interpersonal, communication, and organizational skills
• Possesses empathy, compassion, and nonjudgmental approach to diverse populations
• Excellent problem-solving and critical thinking abilities
• Creative and flexible in finding individualized solutions to complex needs
• Ability to multi-task effectively while managing time and prioritizing responsibilities
• Proficient computer skills (e.g., Microsoft Office Suite, client database systems)
• Knowledge of local community resources, public assistance programs, and housing systems
• Familiarity with trauma-informed care, harm reduction, and strengths-based approaches

Job Specifications

• Medium: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. 100% of the time.
